BH's are a prediction of GR. Einstein at the time he formulated GR, did not realise their consequenc or possibilities. In saying that, way back in 1879 a fellow called John Michell, theorised what he called "Dark Stars" simply based on Newtonian physics. That is, he theorised that if a collapsing star could reach a certain density, where the escape velocity on its surface reached 186,000 MPS, then light would not be able to get away. The consequences though at that time, were just too astonishing to believe and his stuff was pidgeon holed and forgot about. The Dark Star concept though had a surface just below where the light was turned back, unlike the GR version, which has incorporated what we call a "Schwarzchild limit", which is a parameter at which once reached, nothing will stop further collapse, and just happens to coincide with the BH's event horizon. Which gets to your final question.....What's inside a BH?? Absolutely nothing, other then critically curved spacetime, and at the very center a Singularity where all the mass that makes up the BH and what consequently falls in resides. The Singularity is at the quantum/Planck level at which science as yet, cannot probe or investigate.
Your questions are broad and vague, however: He didn't: the idea was largely invented in 1958 and the term coined in 1964, while Einstein died in 1955.

Hi nghia_dtvt5 The poster Farsight has beliefs that at best are not mainstream beliefs, and in part are really just plain wrong. The Dark Star is a Newtonian concept and does not follow the GR prediction of GR, the Schwarzchild limit and compulsory collapse to Singularity status. My brief description is on the mark, I'm pretty sure, even though I'm only a layman. If I was wrong, some more learned astute folk would be taking me to task. Keeping all that in mind, it's best if you google reputable sites as to what a BH is and what the mainstream view is, and don't take my word or Farsight's as gospel. All the best.

Vixra is a non-peer reviewed site that will accept anything regardless of the quality. This paper appears to be typical of the majority of the papers on the site which are basically a mixture of conjecture, a lack of understanding of physics and just plain pseudo-scientific handwaving.
